25th Combat Aviation Brigade Clinic

The 25th Combat Aviation Brigade Clinic provides health care to the Wheeler Army Airfield military community and beneficiaries on the islands of Hawaii through a comprehensive range of health services, including flight medicine, primary care, clinical pharmacy, and behavioral health, within the Soldier Centered Medical Home. The CAB Clinic also provides physical therapy, pharmacy ancillary services, and acute care during normal operating hours. Emergency services are not available. Q: Where are you located?
A:  We are located at 1280 Wright Ave. on Wheeler Army Airfield, next to the flag post and Wheeler Elementary/Middle Schools.

Q:  Are you open on days of no scheduled activities?
A: No; however, sick call services are provided at the SCMH Consolidated Clinic (typically the 8th TSC Clinic, located on the second floor of Building 678, Desmond T. Doss Health Clinic - Schofield Barracks).

Q:  How do I complete my flight physical?

Flight Physical FAQs

Q: How can I start my flight physical?
A:  Two ways: walk in or by phone. In either case you will receive a phone call within three (3) business days with instructions on how to proceed.
Walk-in: Upon walk in, you will be asked to fill out a form with your information.
Phone: You can call our front desk at 808-656-1628 or flight nurse/medic at 808-656-1620 and let them know that you need to do a flight physical Part 1. 

Q: I am new to the island. How do I get a New Duty/Local UPSLIP?
A:  You will need to call the front desk at 808-656-1628 and let them know that you are new to the island and will need a new duty/local UPSLIP. A Telephone Consult will be sent to your unit provider and their care team will contact you within three business days. UPSLIPS are sent to your provider by email.

Q: My Flight Physical has been approved by Fort Novosel. What do I need to do to get an UPSLIP?
A: Provide your full name, phone number, unit/company, and DOD ID number to our front desk by calling 808-656-1628, or in person at the clinic. The provider/medic will contact you within three business days. UPSLIPS are sent via e-mail by your provider.

Q: What should I do if I need an EKG or pilot measurements?
A:  Please come in during our walk-in hours on Mondays, Tuesdays, Wednesdays and Friday afternoons from 1 - 2:30 p.m. Inform the front desk that you need an EKG/pilot measurements and our staff will assist you.

Q: What is my birth month window?
A: Your birth month window is two months before your birth month. For example, if you were born in December, your birth month window starts Oct. 1 and ends Dec. 31. You can begin your Part 1 during this time. AERO recommends you start early due to unexpected circumstances that could occur. Starting the Part 1 early will prevent the need for an extension in the future.

Q: How can I get an UPSLIP extension?
A: Provide your full name, phone number, unit/company, and DOD ID number to our front desk by calling 808-656-1628. The provider/medic will contact you within three business days. Please have a valid reason on why you need one. UPSLIP extensions are sent via e-mail by your provider.

Q: Can somebody else pick up my packet for me?
A: No. Due to HIPAA, we can only give the packet to the person for which it's designated. The packets are made specifically for that person and personal information is located in the packets.

Q: I want to be a pilot, but I'm not in an aviation unit. What are the requirements to start my packet?
A: First, you will need to complete the Selection Instrument for Flight Training test. You will need to have a score of 40 or higher in order to qualify. Please contact the education center at Schofield for the most current information.
Second, you will need a memo from your commanding officer stating that you are required to do a flight physical CLASS 1. Please provide your POC contact information in the memo. If you need a template, please come to the CAB to request one. Further instructions will be provided by staff once these have been completed.
