Soldier Centered Medical Home

About

The Soldier Centered Medical Home is made up of five primary care clinics for routine, non-urgent care and acute health problems to active duty Soldiers. Upon in-processing to Schofield Barracks, each Soldier is assigned to a primary care clinic and a primary care manager, based on the Soldier’s assigned unit.

  • After completing the online portion of your PHA in MODS, schedule your PHA appointment through the MHS GENESIS Patient Portal
