Description of Services

Audiology/Army Hearing Program clinic provides hearing readiness services and hearing protection fittings to Active Duty Service Members and noise exposed DOD Civilians. We provide diagnostic audiological evaluations by referral and appointment only. Diagnostic audiological assessments include vestibular assessments, auditory processing evaluations, hearing aids evaluations and fitting for Active Duty only. Dependents and Retirees are eligible for audiological assessments with a referral from their Primary Care Manager on a space-available basis. Retiree-at-Cost-Hearing-Aid-Program (RACHAP) is available on a space available basis. There is one Active Duty provider and a vacant DOD Civilian provider available to conduct diagnostic evaluations.

Preparing for your visit

1. Please arrive 15 minutes prior to your appointment.
2. Physicals for School/ Warrant Officer Packet/Commission Officer Packets need to have a valid hearing test on record within last 90 days.
3. Chapter and other Separation Physicals need to have a valid hearing test on record within last 6 month.
4. Prior to PCS, a valid hearing test needs to be completed within the last 12 months.

We do not sign or fill out any Physicals/2808/PHA forms. These forms will be filled out by a provider in your Medical Part/Phase 2.

Q. How do I schedule my annual hearing test?
A. Call the central appointment line at (888)-683-2778 to make an appointment
Q. How do I cancel or change my appointment?
A. Call the Audiology front desk at 808-433-8326 during business hours. All appointments need to be canceled at least 24 hours in advance to avoid a no-show.
Q. How can I get another copy of my hearing test?
A. The clinic will provide one copy at the time of testing. Patients can request additional or replacement copies from Troop Medical Records or print a copy from TRICARE Online.
Q. Can I schedule a unit hearing test appointment/hearing rodeo?
A. Call the clinic directly at (808)-433-8326 to inquire about unit appointment availability
Q. How can I get more earpro?
A. Active Duty Soldiers can walk-in to the clinic for individual hearing protection. Earplugs require fitting, so we only issue it to the person who will be using it. Supply NCOs needing to order earpro for their unit should see the Earplug Ordering Information document under the Handouts section below.
Q. How do I read my hearing test results?
A. See “Understanding Your Hearing Test” under the Handouts section below. Call or stop by the clinic if you have specific questions about your results.
