Description of Services
Audiology/Army Hearing Program clinic provides hearing readiness services and hearing protection fittings to Active Duty Service Members and noise exposed DOD Civilians. We provide diagnostic audiological evaluations by referral and appointment only. Diagnostic audiological assessments include vestibular assessments, auditory processing evaluations, hearing aids evaluations and fitting for Active Duty only. Dependents and Retirees are eligible for audiological assessments with a referral from their Primary Care Manager on a space-available basis. Retiree-at-Cost-Hearing-Aid-Program (RACHAP) is available on a space available basis. There is one Active Duty provider and a vacant DOD Civilian provider available to conduct diagnostic evaluations.
Preparing for your visit
1. Please arrive 15 minutes prior to your appointment.
2. Physicals for School/ Warrant Officer Packet/Commission Officer Packets need to have a valid hearing test on record within last 90 days.
3. Chapter and other Separation Physicals need to have a valid hearing test on record within last 6 month.
4. Prior to PCS, a valid hearing test needs to be completed within the last 12 months.
We do not sign or fill out any Physicals/2808/PHA forms. These forms will be filled out by a provider in your Medical Part/Phase 2.
