Exceptional Family Member Program (EFMP)

What is the EFMP?

The EFMP supports families with special medical and educational needs. Each branch has its own EFMP and enrollment
process. You should enroll your family into EFMP as soon as you are considered eligible for enrollment.

EFMP family support services are also available at your installation. The family support services team is your main point of contact for EFMP information.

They can connect you with:

• Community Support Services and Resources

The family support services team can also answer specific EFMP questions based on your branch or installation.

Active duty sponsors who have family members that meet the criteria for specific medical or educational needs are mandated to be enrolled to ensure needed medical or educational services are available as the gaining base and local area.

Q. How do we contact the EFMP office since they are teleworking due to current COVID conditions?
The best method is via their organizational email address at
usarmy.tripler.medcom-tamc.list.pe-efmp@health.mil
This allows the entire EFMP staff visibility and ensures a timely response. Through this email address you can submit your questions and any necessary documents.
The Army EFMP website https://efmp.amedd.army.mil/ also has an interactive map under by clicking on “Overseas Travel” then next on “families in remote locations (not near an MTF)” and this will bring about the U.S. map. Click on the state (twice) and an email box will appear with the servicing Army EFMP email address.
Q. I have orders to another overseas location what do I do now?
Attend the Levy brief, your S1should provide you with a DA form 5888, Part A needs to be completed and your S1 needs to sign off on the AUTHENTICATION part block 8 a-e. Part B should only be completed by an Army EFMP Medical Practitioner and Physician.
Your family members listed on the form will be required to have a current Physical examination or sufficient medical records, within last 12 months to be able to determine one’s current health status.
Q. I cannot get my orders until my EFMP has been processed and approved at my new duty location, how can I find out the status of this?
This action takes place between the Family Travel Offices as they provide the Official travel response. Communicate with your local Family Travel Office and they should be checking on the status.
Q. I need to Command Sponsor my family, what do I do, who can I call?

Q. I received and email from HRC stating that my EFMP needs to be updated, what do I need to do?
Have your family members schedule an appointment with their primary care provider and have them complete a new DD form 2792. Review the document once completed, sign pages 2 and 3 block 9a-c. Ensure DEMOGRAPHICS on page 3 is also completed.
Additionally, it is important that all previously documented diagnosis are addressed on the update. If resolved then it should note as such.
Submit to your local EFMP office to complete update.
Q. My child is enrolled or education who can help with the update?
Please contact your child’s school or the school in your area and they can assist with the DD form 2792-1. For children 3 and older and Individual Educational Plan (IEP) will also be required. If under 3 contact the Early Intervention Services and they can assist with an Individual Family Support Plan (IFSP).
Q. How do I disenroll from EFMP?
Schedule an appointment with your family member's Primary Care Manager and complete a new DD form 2792. Review the document once complete. When you have confirmed the document is correct, sign page 2 and page 3, blocks 9 a-c. You must complete page 3. At the appointment, speak with the provider about your desire to disenroll from EFMP. If your provider recommends disenrollment, submit the forms to the EFMP office. The EFMP office will ensure the request is in compliance with AR 608-75.
Q. How long is enrollment for?
Enrollment is good for 3 years and should be updated anytime in between if there have been any significant changes to a family members medical conditions or requirements.
Q. I was recently denied Family Travel, what can I do about this?
Family Travel denials occur when the screening location upon review of a family members medical conditions and / or requirements exceed the locations capabilities.

You can schedule an appointment with the family members primary care provider and/or specialists and have them review your current enrollment to determine if there are any warranted changes or updates that could assist with an appeal.
Q. I am interested in certain locations is there a list of locations that we can look at to see if our needs are available?
Unfortunately not. Capabilities can change on any given day and without notice. This requires the EFMP office to conduct assessments based on current capabilities.
Q. What is the difference between Army Community Services (ACS) EFMP and the medical EFMP?
ACS EFMP can assist with a number of things that do not fall under medical. They can assist with resource information, with Department of Education questions and issues, housing, with CYS services and Respite Care.

Q. Why does Respite Care provide so little in terms of hours and why are there so many limitations with the types of providers?
There are two types of Respite Care;
-TRICARE under the Extended Care Health Option (ECHO) https://tricare.mil/echo
and
-ACS EFMP, this is a non-medical program that is managed through Army G9 and not medical EFMP. Communicating with ACS EFMP Usarmy.schofield.id-pacific.mbx.acs-efmp@health.mil as they can provide the appropriate guidance
