Description of Services:

• Pre-placement physical exams
• In-processing of DDHC/TAMC/Hawaii Garrison personnel
• Qualification physical exams for firefighters, police officers, and deployments
• Job-related medical surveillance
• Fitness for duty
• Work injury and illness evaluation and treatment
• Respiratory medical clearance
• Hearing and occupational vision conservation programs
• Pregnancy surveillance
• Blood and body fluid exposure follow-up evaluations
• Environmental exposure evaluations with Industrial Hygiene support
• Ergonomic Assessments with Industrial Hygiene, Physical and Occupational Therapy support
• Occupational and health care communicable disease exposure consultations, in partnership with Infection Control & Epidemiology

Preparing for your visit:

• Please bring all necessary documentations from CPAC and your Supervisor.
• Arrive 15 minutes prior to your appointment to allow enough time for check-in.
• You will be considered a “No Show” if you are late more than 15 minutes.
• Allow extra time for parking and walking to the clinic.
• Please call us at 808-433-8391 at least 24 hours in advance for cancellation.
• For child care services during the appointment time, please make a reservation at 808-624-5645 for the Armed Services YMCA Children’s Waiting Room.
